fix: 持久化未接受时仍在内存中推进 lastProcessedAssistantFloor
...
当提取核心成功但持久化未被接受时，之前不推进楼层会导致同一
会话内 pending 模式重复提取已处理的楼层。现在即使持久化未
接受也在内存中推进楼层（不追加 batchJournal，保持回滚完整性）。
重载时 floor 和图谱都会回退到最后持久化状态，保持一致。

fix: 重新提取回滚点不可用时自动执行历史恢复并降级为 pending 模式
...
当提取/整合被中断后手动重新提取（rerun 模式）失败时，不再显示
死胡同错误，而是自动尝试历史恢复后继续提取未处理内容。

- rebindRecallRecordToNewUserMessage: 从 frozenRecallOptions 补全 recallInput/boundUserFloorText/authoritativeInputUsed
- ensurePersistedRecallRecordForGeneration: already-up-to-date 守卫增加 recallInput 非空检查
- 新增 tests/recall-reroll-reuse.mjs 回归测试
